Here's how it works:

Mammogram: $75 or the cost of the test up to $200 - Once per year

Pap Test: $50 or the cost of the test up to $200 - Once per year

One more test from the above list with a set benefit of $50 or $75 or $100 - this amount is determined by the level plan you have, example: Basic, Enhanced or Premier - Once per year

Getting Paid: Submit a copy of your bill from the doctor or facility for the mammogram and pap to get the most money from your test. On the other tests, you can submit something on the doctor/facility's, letterhead - and you get a check in the mail!
