Promote Early Detection - Get Paid to Get Tested!
Can you think of a bigger motivator to get your annual wellness tests? The wellness benefits on Allstate's cancer insurance plans are second to none! Women get paid for three annual wellness tests, mammogram, pap and one other listed test (see below) and men get paid for one annual test - see below for other wellness tests. So you can never be diagnosed with cancer but still have this policy pay for itself with this feature alone! Here's an example of how the plan works in California:
$200 - The cost of the test or max $200 paid for annual mammogram
$200 - The cost of the test or max $200 paid for annual pap test
$100 - Paid on Premier plan for one more test - see below for list.
$500 - Max paid out in just wellness tests! Cost for the year - $362 (based on age 48 with the Premier Individual plan and ICU coverage included)

Mammogram Test. We pay actual charges of test up to $200 for a covered person for women ages 35 on up - paid out once per calendar year. If charges can not be obtained than we pay $70.

- If your doctor has given you a presciption for this test than this is considered recommendation.

Pap test or cervical cancer test. We pay actual charges up to $200 for an annual test. This test is limited to one test per covered person. If specific charges can not be obtained we will pay $50.

- If you have a family plan then your daughter would be eligible as well.

Other wellness tests listed for both men and women. Women can have the two above tests plus one more from this list annually. Men will receive money for one of the bottom tests below, per year. Please note, the pay out is based on the plan you pick. Example: Basic - $50. Enhanced - $75. Premier - $100.
- Biopsy for skin cancer (could be a mole being removed)
- Blood test for triglycerides and bone marrow testing
- CA105-3 (blood test for breast cancer), CA125 (blood test for ovarian cancer), CEA (blood test colon cancer)
- Chest X-ray
- Colonoscopy
- Doppler screening (vascular disease), EKG, Echocardiogram
- HPV (Human Papillomavirus) vaccination
- Lipid Panel (Cholesterol count)
- PSA (prostate cancer blood test)
- Stress Test on bike or treadmill
